Mid-Term Appraisal Due Dates:

• Spring-Summer, mid-term: June 27, 2008
• Summer-Fall, mid-term: September 26, 2008

The Board Member will ensure that the four steps below are completed by the due dates:

2. Board Member Affidavit:
Mail or fax this completed affidavit to the MECOP office.

3. Mentor Appraisal:
Do not send this appraisal to the MECOP office unless the performance was unsatisfactory. After the appraisal meeting, you may keep it for the company records.

4. Intern Appraisal:
Do not send this appraisal to the MECOP office unless the performance was unsatisfactory. After the appraisal meeting, you may keep it for the company records.
